      Make a complaint online

      Before starting your secure, online complaint, please read what to do before making a complaint. This will help:

      • confirm we are the correct organisation to handle your complaint
      • assess your case as quickly and efficiently as possible.

      Privacy

      Everything you submit, including your correspondence address, will be shared with the people or organisation you are complaining about. If you do not want to share your personal address, consider using an email address or PO Box, or contact us to discuss your options before submitting.

      Using the form

      The form is interactive and will show only the questions that are relevant to your answers. The form may take up to 30 minutes to complete.

      What to include on the online form

      You will be asked to provide:

      • your contact information, including your name and correspondence address
      • the names of the person/s or organisations you are complaining about
      • a summary of what you are complaining about and when it happened. You can enter this directly into the complaint form, or attach a document with this information
      • a copy of a payslip if you are complaining about your employer.

      After you submit the form

      After you submit your online complaint, you will receive confirmation it has submitted successfully and an email to confirm we have received your complaint.

      Unable to submit form in one sitting

      If you cannot complete the online complaint form in one sitting, you can download the word version, save it to your device or print it and complete at a time that suits you.
